2019-11-16 · Scapholunate ligament tear is most frequently encountered in the aftermath of trauma in supination and extension of the wrist. It generates chronic instability, leading to osteoarthritis. It may be associated with fracture of the distal epiphysis of the radius or of the scaphoid.

A static instability is generally readily visible, but a dynamic scapholunate instability can only be seen radiographically in certain wrist positions or under certain loading conditions, such as when clenching the wrist, or loading the wrist in ulnar deviation. scapholunate dissociation causes the scaphoid to flex palmar and the lunate to dorsiflex if left untreated the DISI deformity can progress into a SLAC wrist DISI deformity may also develop secondary to distal pole of the scaphoid excision for treatment of STT arthritis DISI is a form of carpal instability dissociative Traumatic injury of wrist while in extension resulting in disruption, most commonly, of the dorsal scapholunate ligament. This is also known as a DISI (Dorsal Intercalated Segmental Instability) injury. (1,2) Exam: Inspection – may notice some edema. Palpation – swelling, or occasionally ganglion cyst in older injuries. Scapholunate injury (or scapholunate dissociation, SLD) is one of the most common and significant ligament injury of the wrist.

Sometimes the athlete may not recall the fall because they didn’t seek treatment immediately, but instead sought treatment weeks later when they continued to have pain. A scapholunate torn ligament does not heal on its own. If left untreated, a torn ligament typically leads to arthritis over time. This is because the bones of the wrist are no longer held in normal alignment. The goals of treatment are to decrease pain and prevent arthritis.

Hand and wrist injuries commonly occur and can be debilitating. Scapholunate dissociation generally requires surgery, and if left untreated, can lead to arthritis in the wrist1. The overall goal of this research is to make a positive impact on the assessment and treatment of wrist injuries and on prevention of osteoarthritis as a result of injury.

With the arthroscope in the midcarpal ulnar portal and the probe in the midcarpal radial portal for the left wrist, the stability of the scapholunate joint is checked. (L, lunate; S, scaphoid.) Table 1.

Scapholunate dissociation of left wrist

A person who experiences pain in the back of the wrist or the thumb side of the wrist (following a fall) should be examined for scapho-lunate dissociation. Our hands (right beside the area of the wrist) have many different bones in it. Scapholunate dissociation, also known as rotary subluxation of the scaphoid, refers to an abnormal orientation of the scaphoid relative to the lunate, and implies severe injury to the scapholunate interosseous ligament and other stabilizing ligaments. Carpal dissociation implies carpal instability, which has important clinical implications; thus it is essential to identify this finding on imaging. Scapholunate dissociation results from an injury to the scapholunate interosseous ligament which joins the scaphoid and lunate wrist bones together.

Multiple surgical procedures have been designed to reduce the scapholunate interval, restore normal wrist What is a Scapholunate Ligament injury? Ligaments are strong bands of fibrous tissue which connect bones to bones. The scapholunate (SL) ligament connects the scaphoid bone to the lunate bone in the wrist. It is as important as the ACL in the knee. The scaphoid and lunate bones are … 2020-01-29 Scapholunate Dissociation.
